Coffee

Coffee covers sleepiness. It doesn't remove it. If you're sleepy: a coffee, then twenty minutes' sleep, then go.

It's nine. The moka pot is on the stove, as it is every morning. Coffee is the ritual that opens the day. At the café, standing at the counter, the words are always the same: a coffee, please. Then we set off again. The need for sleep has a molecule: adenosine. It builds up through every hour we are awake. Caffeine has a similar shape and takes its place. The signal is covered. But adenosine is still there, waiting. Motorway, at night. After a strong coffee, 75 per cent drive as if it were daytime. After a half-hour nap, 66 per cent. After a decaf, 13. Coffee doesn't clear quickly: every five hours, half of it is still there. The nine o'clock cup is down to an eighth by midnight. The five o'clock cup, to more than a third. It isn't just coffee. Tea, cola, energy drinks, chocolate, painkillers: the total goes to bed with you. Anyone who sets off after a coffee feels awake. The need for sleep hasn't gone: it's only covered.

It's nine. The moka pot is on the stove, as it is every morning. Coffee is the ritual that opens the day.

At 9, the body's alertness is climbing towards its late-morning peak (episode 04). Coffee arrives at the hour when it is needed least: it is a ritual before it is a stimulant, and that is fine. In Switzerland people drink over 1,200 cups a year each, among the highest in the world. This episode's images are generated and set in Lugano, declared as an exception: a recognisable Switzerland, not just anywhere.

At the café, standing at the counter, the words are always the same: a coffee, please. Then we set off again.

At the counter you ask for a coffee: in Ticino and Italy the word espresso is not said, it is drunk. Standing, in a minute, often with a glass of water beside it. Ristretto, lungo, macchiato, corretto: the names change, the dose does not: about 80 mg of caffeine in an espresso (EFSA 2015). It is the picture of the most common belief: a coffee and off I go.

The need for sleep has a molecule: adenosine. It builds up through every hour we are awake.

Adenosine is the residue of the energy cells burn (it comes from ATP). While we are awake it builds up in the brain and docks onto its receptors, mainly A1 and A2A: the more adenosine docked, the more sleepiness. During sleep it is cleared. It is one of the two forces of sleep, the pressure that grows with hours awake; the other is the circadian clock (episode 04).

Caffeine has a similar shape and takes its place. The signal is covered. But adenosine is still there, waiting.

Caffeine has a shape similar enough to fit the same receptors without activating them: it occupies the lock and adenosine cannot get in. The signal is covered, not switched off: adenosine keeps building up behind the door. When caffeine fades, it arrives all at once and sleepiness comes back stronger. People who drink coffee every day make more receptors, and the same cup does less: that is tolerance (Nehlig 2018; EFSA 2015).

Motorway, at night. After a strong coffee, 75 per cent drive as if it were daytime.

Twelve young men drove 200 km on a real motorway between 2 and 3.30 at night, with an instructor on board. After half a cup of coffee with 200 mg of caffeine, 75% drove as they would by day, that is with no or only one lane departure (Philip 2006). It is one of the few open-road studies: the figure transfers to real driving. Pierre Philip is also one of the Monte Verità voices.

In the same study, after a 30-minute nap in the car 66% drove as they would by day; after a decaf (15 mg), 13%. Coffee and the nap both work, and the following night's sleep does not suffer. Limits stated by the authors: a single dose, a single nap length, only rested young men (Philip 2006).

Coffee doesn't clear quickly: every five hours, half of it is still there.

Caffeine does not end, it halves. The half-life in a healthy adult is about 5 hours, but ranges from 1.5 to 9.5 depending on the person (Institute of Medicine 2001): genes, smoking (shortens it), the contraceptive pill (doubles it), pregnancy. 95% is cleared by the liver through the enzyme CYP1A2 (Nehlig 2018). After 5 hours half is left, after 10 a quarter, after 15 an eighth.

With a 5-hour half-life, the 9 o'clock coffee is down to an eighth by midnight: almost gone. The 5 o'clock one, seven hours later, is still above a third (about 38%). And it disturbs sleep: 400 mg taken six hours before bed removes more than an hour of measured sleep, without the sleeper noticing (Drake 2013). Hence the widespread feeling of sleeping well even after an evening coffee: sleep gets worse, but it is not felt. Add the tolerance of daily coffee drinkers and, for a minority carrying the fast CYP1A2 variant, clearance within a few hours (Nehlig 2018).

It isn't just coffee. Tea, cola, energy drinks, chocolate, painkillers: the total goes to bed with you.

Caffeine per serving (EFSA 2015): filter coffee 200 ml 90 mg, espresso 80, energy drink 250 ml 80, tea 220 ml 50, cola 355 ml 40, dark chocolate 50 g 25. Some painkillers contain it too. For an adult, up to 200 mg at once and 400 a day raise no concern (EFSA 2015): an espresso at the café, a cola at lunch, an energy drink at 5 and a painkiller in the evening go past that without anyone noticing. Carlos Schenck, one of the Monte Verità voices, talks about exactly this: caffeinated drinks in the evening and overstimulation that reaches the road.

Anyone who sets off after a coffee feels awake because the signal is covered, not because the need for sleep has passed. It is the same trap as in episode 08: the sleepiness you feel and the sleepiness that is there do not match (Van Dongen 2003). Coffee helps for a stretch; then the debt turns up, often at the worst hour.

What helps

Coffee first, then sleep: it sounds backwards, but caffeine takes 20 to 30 minutes to reach the brain. In the first minutes it does not stop you dozing off, and by the time you wake it is entering the bloodstream: the two effects add up. In a simulator, 200 mg of caffeine followed by a nap of under 15 minutes cut incidents to 9% of placebo, against 34% with caffeine alone; the effect is there even in those who only doze with their eyes closed (Reyner and Horne 1997). The nap stays under 20 minutes, so as not to enter deep sleep. If you are sleepy before setting off, the coffee is not the start: the nap is.
